What are the risks of heart disease in rheumatoid arthritis?
Rheumatoid arthritis (RA) is a chronic, progressive, systemic disease Autoimmune for which there is no cure in progress. This is a common disorder that affects more than 2.1 million Americans.
Patients with complications RA outside the joints, what is called extra-articular disease are particularly at risk of early death. Extraarticular problems that can be seen in RA include skin ulcers, anemia, eye inflammation, inflammation and lung damage, heart disease, inflammation of blood vessels, and rheumatoid nodules (these bumps consisting of inflamed tissue that develop in certain areas, such as fingers, elbows, heels and back of the skull).
A predictor of the presence of extra-articular disease is the level of rheumatoid factor in blood. The higher the rheumatoid factor, the more likely a patient has an extra-articular disease.
A marker newer blood, anti-CCP (anti-cyclic citrullinated peptide) also seems to predict an increased risk of extra-articular problems and more serious diseases.
The bottom line is that RA is not a benign condition. In fact, bears the same mortality of heart attacks first, untreated diabetes, and stage 4 Hodgkin's disease, in some studies. RA should be diagnosed and treated aggressively.
Probably the main cause of death is an increase in cardiovascular mortality. The Estimates range from 33-50% of all deaths in patients with rheumatoid arthritis are due to cardiovascular causes. Patients with rheumatoid arthritis are at significantly increased risk for atherosclerosis, congestive heart failure, stroke, and vasculitis (inflammation of blood vessels).
So What are some risk factors that are particularly important?
These include:
• Severe RA
• Severe extra-articular disease
• Elevated blood levels of homocysteine, that is exacerbated by therapy with methotrexate
• disorders of the blood vessel walls
• Drugs that are used to treat RA such as corticosteroids and nonsteroidal anti-inflammatory drugs
• The overproduction of inflammatory cytokines (chemical messengers of the disease)
• rheumatoid nodules
A note on drugs …
Methotrexate, the drug most commonly used disease-modifying anti-rheumatic is associated with a significant reduction incidence of cardiovascular death. Unfiortuantely, nonsteoridal anti-inflammatory drugs that are commonly used to treat the symptoms of RA increase the risk of blood clots, diseases cardiovascular disease, and congestive heart failure.
The role of other medical problems such as blood sugar, high fat, and so on are a topic for another article.
Recommendations for reducing cardiovascular risk in patients with rheumatoid arthritis are:
• Low doses of aspirin (which should be completed some form of protection of the stomach) particularly in patients taking nonsteroidal anti-inflammatory drugs.
• statins for people with high lipid
• folic acid supplementation to minimize the high homocysteine levels seen in RA
• Caution with use of TNF-inhibitor in patients with congestive heart failure
• Control of co-morbid conditions such as smoking, diabetes and hypertension which may aggravate the tendency towards cardiovascular risk.
Complicating matters is some data showing that TNF inhibitors can raise lipids serum in some cases. The implications of this phenomenon on the overall picture is still unknown.
A bright note: there is increasing evidence that aggressive treatment methotrexate and TNF inhibitors reduce the incidence of cardiovascular events. Therefore, the problem of lipids above may not be so important. Further studies are necessary, of course, to confirm these results.
